Create or Edit a Consent Form

Step 1: 

Log in to Growth99 using your login credentials and navigate to the Templates Section under the Marketing main menu.

1. From the template dashboard, go to the  "Consent" section.
2. Click on "Consent" from the available options.

Step 2: Use or Edit Existing Consent Forms

Growth99 comes with a set of pre-loaded default consent forms for general procedures.

• You can review these existing forms, edit them to fit your clinic's needs, and save the updated version.
• It is a great way to save time while still customizing the content to match your branding or legal requirements.

Step 3: Create a New Consent Form (Optional)

If you need a completely custom form:

1. Click the “Add Template” button.
2. Enter the title ‘Name’ of the consent form (e.g., “Chemical Peel Consent”).
3. Add any tag or specialization if required (optional)
4. In the Body section, add the content of your consent form.
5. You can use available dynamic variables (such as patient name, date, Clinic name, and address) to personalize the form automatically. These variables help populate patient-specific details while using the form.
6. Format the text using headings, paragraphs, checkboxes, etc.
7. Click “Save” once your form is ready.
