Configure Consent Forms and Questionnaires

Growth99 supports a Seamless Patient Experience by allowing patients to view and complete consent forms and questionnaires immediately after booking an appointment.  This feature ensures all necessary documentation is submitted as part of the booking workflow, streamlining the patient journey. Follow the steps below to enable and configure this feature for your business.

Step 1: Log in to Your Growth99 Account

Log in to your Growth99 business account using your credentials.


Step 2: Access Locations

On the Business Settings page, click “Locations.”

Step 3: Edit Your Clinic Location

On the Locations page, find your clinic and click the “Edit” button under the Actions column.


Step 4: Navigate to the Configuration Tab

In the Edit Location view, go to the “Configuration” tab.

Step 5: Enable the Seamless Experience Option

Check the box labeled:

“Enable submission of consent forms and questionnaires in the booking workflow" to enable the seamless patient experience feature for the selected clinic location.

Step 6: Attach Questionnaires and Consents to Services

From the Business Settings menu, click on “Services.”

Click the “Edit” button under the Actions column for the desired service.

Select the appropriate questionnaire from the Questionnaire dropdown list.

Click “Save” to apply the selection.

Repeat the same process under the “Consent” section to attach relevant consent forms.

Step 7: Test the Seamless Patient Experience

To test the configuration:

  • Visit your Growth99 public booking link.
  • Book a test appointment.
  • After booking, the Questionnaire and Consent forms will automatically appear.
  • Once the patient has filled out and signed the documents, they should select “Submit” to complete the submission process.


Step 8: Review Submitted Forms

To view submitted questionnaires and consents:

  • Go to the “Contacts” section.
  • Select the customer profile.
  • On the Contact Details page, view the completed forms as shown below.
